Your Popula nurse helps you set up and understand your connected device — blood pressure cuff, pulse oximeter, glucose monitor, weight scale, or other appropriate device.
Readings transmit automatically
When you take a reading, it transmits securely to Popula. Automated reminders help you stay consistent.
Nurse review
A Popula nurse reviews flagged readings and patterns according to the member's monitoring plan and established workflows — not just individual data points.
Outreach when appropriate
When a reading or pattern meets established review criteria, your Popula nurse reaches out to gather additional information and determine the appropriate next step within Popula's scope.
Healthcare team connection
When appropriate, Popula helps connect relevant information to your healthcare team.
Closed loop
Follow-up is tracked until it's complete — so important information doesn't disappear.
Devices are selected based on your needs.
Popula does not prescribe devices. Connected monitoring is used when clinically appropriate and selected in coordination with your care plan.
